Washington State Elevating Maternal Care Through Highest Doula Reimbursement

By Demetrius Geiger, MPH, CHC Program Manager

The strides taken by Washington State to elevate maternal care standards through increased Medicaid reimbursement rates for certified doulas are truly commendable and forward-thinking. This initiative not only signifies a commitment to equitable healthcare access but also acknowledges the invaluable support that doulas offer expectant individuals on their journey to parenthood.

The decision to allocate $3,500 per birth for doula services marks a substantial move towards equitable compensation for essential birth advocates. It recognizes the importance of comprehensive support before, during, and after childbirth. By providing a sustainable wage for doula services, Washington State is driving a paradigm shift toward the enhancement of the perinatal workforce and addressing disparities in maternal health outcomes.

I am excited about these advancements, and CHC: Creating Healthier Communities supports these efforts aimed at improving access to crucial doula services, similar to our The Black Birthing Initiative, which plays a pivotal role in reducing the Black maternal mortality rate. Washington State has demonstrated a commitment to improving both the quality of life for doulas and birth outcomes. As we celebrate these advancements aimed at improving access to crucial doula services, it is essential to recognize that there remain opportunities for growth and improvement. Ensuring that both doulas and clients can navigate the system with ease, addressing bias towards doulas in healthcare settings, and fostering synergy and inclusivity in recognizing the impact doulas have on positive birth experiences and outcomes are crucial next steps. By continuing to prioritize and support initiatives that promote health equity and elevate the role of doulas in maternal care, we can work towards creating healthier communities where every individual has the opportunity to thrive.

The establishment of a doula hub and referral system showcases the proactive measures to ensure the effective facilitation of doula services statewide. Through investments in infrastructure and support mechanisms, Washington is laying a foundation for a care model emphasizing collaboration between healthcare professionals and doulas.

Washington’s approach establishes a national benchmark for doula reimbursement, with the hope that other states will be encouraged to follow suit by implementing and raising Medicaid reimbursement rates for doula services. It is also crucial for all insurance types to cover doula services to ensure that everyone has access to this important form of support. Doulas are agents of change for all birthing people, particularly for Black women and birthing individuals who face higher rates of adverse pregnancy outcomes regardless of their socioeconomic status. The work done through the Black Birthing Initiative at CHC: Creating Healthier Communities and its partners is vital in ensuring that Black women and birthing individuals receive the support they need. By matching individuals with doulas and ensuring equitable compensation for their services, we are taking important steps toward addressing health disparities and promoting health equity in our communities.
