Unite Us and CHC Partner in Ohio to Remove Social and Structural Barriers to Care to Improve Community Health

With a shared focus on community health and equity, Unite Us and CHC: Creating Healthier Communities recently partnered to identify, recruit and engage community-based nonprofit organizations in the Northeast and Central Ohio regions. CHC will invite community-focused health and social service providers to join the Unite Ohio coordinated care network. The Unite Ohio network aligns with CHC’s mission to address the barriers to good health and equity and create resilient communities where everyone has the opportunity to thrive.
Unite Us offers a technology-based, coordinated care network that enables health and social service care providers across communities to connect with one another for seamless referral and service delivery. The Unite Us Platform is available at no cost to community service providers in Ohio, thanks to support from MetroHealth Systems and Cleveland Clinic, who are already using the Unite Us Platform to connect patients with community-based organizations. The Unite Ohio network offers an easier way to refer and connect clients to the services they need, with increased collaboration and better health outcomes, plus a wealth of data and impact measurements to address health equity and community goals. Most importantly, the Unite Us Platform helps communities better connect health and social care, which helps address health equity issues.
To launch the CHC-Unite Us partnership, CHC held its first hybrid event on 8/26: “The Future of Health: Equity in All Services.” This community conversation is part of an ongoing series of “Future of Health” convenings CHC has hosted over the past few years to promote health equity and the social determinants of health. The Cleveland Cavaliers and Rocket Mortgage Fieldhouse hosted the August “Equity in All Services” event, with speakers from Unite Us, MetroHealth Systems, KPMG, Cleveland Clinic, Ohio Department of Health, Cleveland Department of Public Health, Cleveland Office of Minority Health and more.
