CHC: Creating Healthier Communities exists so that every person in every community has an opportunity to live their healthiest life.
We are a national nonprofit that helps organizations achieve their missions by bringing communities, nonprofits, and businesses together. Our goal is to improve health and well-being through collaboration. Through our unique approach, we work closely with partners to create scalable, sustainable solutions that drive real, lasting change.
Social determinants of health (SDOH) such as housing, food, education and environmental conditions affect as between 30-55 percent of health outcomes. Further, differences in health outcomes and access to care in the U.S. cost approximately $320 billion today and could exceed $1 trillion in annual spending by 2040. CHC offers program development and strategic community alliances to tackle root causes and systemic issues. We center and ground our work in the lived experiences of those closest to and knowledgeable of health outcomes because we believe communities hold the answers to solving them.

CHC is working towards greater health outcomes by building partnerships within communities in ways that intentionally share power and decision making and allow for meaningful participation. Our work towards sustainable change includes alliances with community partners, public agencies, and broader public health movements.
The Elevance Health Foundation provided CHC a 3-year grant focused on addressing the high rates of preterm birth among Black women in the US.
