Terms and Conditions

NO PURCHASE IS NECESSARY TO ENTER: No purchase is necessary to enter or receive a prize and a purchase will not increase your chances of winning the raffle.

ELIGIBILITY: You must be 18 years or older to enter into the gift card raffle. The raffle is only open to people who completed the Black Birthing Initiative Survey and submitted information to be entered into the raffle.

HOW TO ENTER: To be entered in the raffle for the gift card, after completion of the Black Birthing Initiative Survey, you must consent to be directed to a separate page to enter your name, email address, and phone number. Each entrant is allowed to enter themselves into the drawing one time during the raffle period. By participating, you are agreeing to the terms and conditions outlined here.

ENTRY PERIOD: Entry for the raffle will be accepted from May 2023 through October 2024. Five (5) winners will be randomly selected monthly throughout the entry period.. The odds of winning will depend upon the number of eligible entries received. CHC: Creating Healthier Communities reserves the right to extend the raffle period.

PRIZE: The prize for the raffle is a gift card. A total of 85 gift cards will be given away during the entire raffle period.

WINNERS NOTIFICATION: Within five (5) business days following the end of the monthly raffle period, each winner will be notified by CHC: Creating Healthier Communities by email and/or phone call. Each winner will have 7 business days to claim the gift card. Once the winner confirms to receive the gift card, CHC: Creating Healthier Communities will email a gift card linkto the winners. If the gift card is not claimed within 7 business day or if any notification is returned undeliverable, the winner will be deemed to have forfeited and CHC: Creating Healthier Communities is no longer obligated to email the gift card. The gift card is not transferable and there is no cash alternative.

PRIVACY: Each raffle entry will require personal information to be provided. All personal information related to raffle entries will remain confidential.
